Pet License Application Form
Licenses for dogs run from January 1st to December 31st and are due annually. Licenses obtained after March 1st will be assessed $5 penalty per license.
Parade Permit Application
No parade shall be conducted without first obtaining a parade permit not less than two weeks before the date of the parade.
Street Closing Request Application
A Street Closing Request must be completed for any temporary closing of a street for use during an event, festival or other activity.
Citizen Complaint Form
Any individual wishing to file a complaint against an officer of the Charles City Police Department needs to complete a Citizen Compliant Form and submit the form to the Chief of Police.
