1. What forms of identification verification do I need to apply?

You will need to have a state certified birth certificate for any minors in the household (a receipt/proof of one on order will be accepted until document arrives), a social security card for all household members listed on the application (print out from an agency with name and number listed will be accepted or a receipt/letter stating one is on order will be accepted until card arrives), and a state issued picture identification for all household members over 18 years of age, and proof of preferences.
